Home and Cardiac Support Care

LECC launched its Home Visit/Care and Cardiac Support Care at-home in 2019. It is ran by a five-man team of qualified health professionals who are dedicated to catering for the health needs of critically ill patients who require extra attention and support outside the hospital.

Certain patients walk into LECC for care that can be more conveniently and affordably given at home by our dedicated team.

When a loved one’s health declines, it is stressful for families. Worries about falls, medications, rehabilitation, and hospitalization can take their toll. Above all, families are concerned about their loved one’s recovery, and it’s difficult to know what the best solution is to help them do this. If this sounds familiar, you are not alone.

Often, the best option is to bring the care to your loved one so they can recover where they feel most comfortable: at home. Patients who receive skilled medical care at home can avoid unnecessary hospital admissions and high medical bills.

LECC dedicated and trained home care staff offer individualized care that can help your loved one get back on their feet and regain their independence in familiar surroundings.

What if my loved one’s health is not declining? That is fine. Sometimes, its just a matter of old age and taking the necessary precautions.

How do you know if home health care is right for you?

Navigating your options for care can be confusing and overwhelming. When you call LECC, you’ll request to speak to the head of home care, Oluwakemi — one who can knowledgeably answer your questions about you or your loved one’s health challenges and help you make choices you feel good about.

We begin by understanding the level of support you require and the agreed goals between the provider or physician and the patients based on symptoms and challenges. We then develop a care plan that is suited for your specific needs and goals. Although each care plan is different, most our plans address these issues:

  • Complete health assessment
  • Medication and side effect management
  • Symptom management
  • Disease management
  • Coordination of in-home care and support services
  • Individualized therapy to increase independence
  • Safety education in the home
  • Health education
  • Pain assessment and management
  • Coordination of care with other members of the health care team including specialists and family
  • Diet and nutritional education and management
  • 24/7 support

LECC’s home care and cardiac teams will help your loved one stick to their physician’s care plan—and achieve their own personal health goals. With our care team’s support, your loved one can improve their ability to independently manage their medical condition, restore their pre-admission level of activity, and avoid readmission to the hospital and other conditions such as stroke and heart attack.
